Health Requirements

Prairie Hill School has received approval of its plan to require physical examinations and current immunizations of entering kindergarten and fifth grade students before school begins in the Fall. The school should receive documentation BEFORE the first day of school. Any student not in compliance with these requirements will be unable to attend school until the requirements are met and documentation is received.

All New Students (including kindergarten students): Birth Certificates

Because of a 1987 Illinois state law, Prairie Hill School is required to have a copy of the certified birth certificate within 30 days of first enrollment. A certified birth certificate is one obtained through the County Clerk's office in the county of birth. It will usually contained a raised seal. While the hospital's copy is a nice memento, only the certified copy can be accepted.

Kindergarten Entrance Requirements

+ Be 5 years old by September 1st

+ Have a current physical examination on file. A current physical is one obtained within one year prior to school entrance. This physical must be recorded on an Illinois form.

+ Current immunizations

+ Have a dental examination on file

+ Provide an official copy of the birth certificate

+ A lead assessment. This assessment requires that a health care provider ask the parent seven questions about possible or potential exposures to lead. If any of the answers are yes, the health care provider is then required to perform a lead screen, which requires that blood be drawn from the student. ****Many of our students do not have this recorded on their physical exam. Please be sure to ask your health care provider to record this information on the physical form.****

Fifth Grade Entrance Requirements

+ Have a current physical examination on file. A current physical is one obtained within one year prior to school entrance. This physical must be recorded on an Illinois form.

+ Have a dental examination on file

+ Have received the Hepatitis B vaccine. This is a series of three injections spaced over a six month period. (Since this requirement takes time to complete, it is suggested that injections be started early in the fourth grade year.)

+ A valid physical examination must be on file in the health office before participation in practices or try-outs for the sport. A valid sports physical is one obtained within one year prior to participation.
