Prairie Hill CCSD #133 requires that physical examinations, proof of up-to-date immunizations, dental and eye exams for all students new to the district, entering kindergarteners, and sixth grade students be on file with the nurse before school begins in the Fall. The school should receive documentation BEFORE the first day of school. Any student not in compliance with these requirements will be unable to attend school until the requirements are met and documentation is received.

All New Students (including kindergarten students): Birth Certificates
Because of a 1987 Illinois state law, Prairie Hill School is required to have a copy of the certified birth certificate within 30 days of first enrollment. A certified birth certificate is one obtained through the County Clerk's office in the county of birth. It will usually contain a raised seal. While the hospital's copy is a nice memento, only the certified copy can be accepted.
